You can begin by requesting an appointment through our website. We will reach out to begin the pre-appointment intake process. This will include providing you with access to our online patient portal where you will fill out some basic information and consents. Your appointment will take place within that same portal utilizing HIPAA-compliant video-conferencing software. During the intake, we may request additional information.

Please do your best to gather the following information to prepare for intake:

  1. Any previous genetic testing results

  2. A copy of any clinical or pathology reports from any major biopsies or surgeries

  3. As much as available, your biological family’s health history; grandparents, parents, aunts, uncles, first-cousins, siblings, and children

    • If any relatives have had genetic testing, a copy of the results may be useful

      • Important details include: year of the test, lab name, genes tested, and as detailed results as possible.

  4. A copy of your payment method and possibly insurance card if genetic testing is requested.
